Answer · A short answer
How to cut no-shows without overbooking
Confirm every appointment 72 business hours before it, release the slot if nobody confirms by 48 business hours, and give the freed slot to someone on a cancellation list. You fill the schedule with patients who said yes, so there is nothing to overbook.
Why overbooking backfires
Overbooking bets that some patients will not come. When they all do, the waiting room fills, clinicians run late and the patients who turned up on time pay for it. When the bet goes the other way, you still have empty slots.
It also hides the real problem. You never learn which patients were not coming, so you never get the chance to give their slot to someone who would.
Confirm early
Contact every patient 72 business hours before their appointment. Count business hours only: a Wednesday appointment is confirmed on the Friday before. That is early enough that a released slot can still be filled.
Call first. If nobody answers, leave a short voicemail and send an email straight after, so the patient can confirm by replying.
Release on time
If there is no confirmation by 48 business hours before the appointment, release the slot. For that Wednesday appointment, the cut-off is the end of Monday. Tell the patient they will be offered the next open slot.
Move a released patient at most twice. After that, return the referral to the doctor who sent it, so nobody waits in a loop.
Fill from a list, not a guess
Keep one cancellation list of patients who would come sooner. When a slot is released, offer it to the most urgent referral first, then to the list. On quiet days, work the list until the week is full.
Expect Mondays to be busier while releases are rebooked, and plan an extra hour for it. The result is a schedule of people who said they are coming, and room for urgent patients who would otherwise wait.
