Answer · A short answer
How to fit urgent patients into a full schedule
Make room before you need it: confirm appointments early and release the ones nobody confirms. Keep a cancellation list. When an urgent patient arrives and nothing is free, move one routine patient to another day and put them at the top of the cancellation list.
A full schedule usually has gaps
A schedule that looks full on Monday often has empty slots by Thursday, because some patients do not come. Those slots are lost unless you find out early enough to give them away.
Confirm every appointment 72 business hours ahead and release the slot if nobody confirms by 48 business hours. Each release is a slot you can offer to an urgent patient.
Keep a cancellation list
Keep one list of patients who would come in sooner if a slot opens. When a slot is freed, offer it to the most urgent referral waiting first, then to the list.
On days with no urgent referrals, work the list to fill the week. A schedule kept full this way has room to give when you need it.
When there is truly no room
If an urgent patient must be seen and every slot is confirmed, move one routine patient to another day. Choose someone whose care will not suffer from a short wait, call them yourself, explain why, and put them at the top of the cancellation list so they are the first offered any opening.
Most families accept this when they are told plainly that another patient needs the slot more and that they will be first in line.
Keep a person in charge
Have one person approve every move of a booked patient, and limit moves to two per patient. After that, the referral goes back to the doctor who sent it with a note. These limits stop anyone being pushed forward forever.
Done this way, urgent patients are seen inside the deadline and the schedule stays full. One clinic used this routine to stay within a 14-day rule for urgent referrals.
The routine written as rules you can copy. Copy the confirm-or-release rules.
